Before beginning therapy services the following forms must be completed:

Attendance and Absences
- Therapy will be scheduled for, at minimum, one session per week. Additional therapy sessions each week can be scheduled as needed, if space is available.
- The office is closed for some holidays and professional conferences. Parents will be notified of any days off or vacation days at least one week prior to the date. If your child will be unable to attend a therapy session for any reason, please contact the therapist either by phone or email no later than 9:00 am (7:30am for 8:00am appointments) the day of your child’s appointment. If notice is received later than 9:00 am, clients will be responsible for the $80 absence without notice fee.
- In emergency situations (snow, earthquake, motor vehicle accident, etc.) prior notice is not required and the absence without notice fee will not apply.
- Insurance companies will not be billed for no-show or unreported absences. Parents/individuals will be fully responsible for such charges.
- Allowances will not be made for late arrivals, as sessions will end at the scheduled time and will be charged as a full session.
- If your child is sick, please keep them home. Children, who are not feeling well, will not do well in therapy. Do not bring your child to speech if they are suffering from any of the following: fever, vomiting, diarrhea, flu or flu-like symptoms, severe cold symptoms (persistent cough, unmanaged runny nose with green or yellow discharge, sore throat), skin infections (e.g. impetigo), or pink eye (must have 24 hour on antibiotics minimum). Your child must be free of symptoms for 24 hours prior to returning to therapy and/or have received a negative COVID test if applicable. If the therapist determines they are not healthy enough to participate in therapy, she may call the parent/caregiver to take the child home early. If your child starts feeling sick the day of their appointment, it is recommended for the parent to call as early as possible and cancel your child's appointment. If your child has been exposed to a highly contagious illness such as COVID-19, fifth's disease, chicken pox, or hand, foot, and mouth disease, please call or email to discuss options.
